Collision and Incident Report Requests

The following Durham Regional Police Reports are available to individuals directly involved in an incident or their representatives (with consent) including insurance companies and law firms.
• Motor Vehicle Collisions
• Thefts
• Property damage
• Break and enters

Access to reports for domestic incidents, child custody, or assaults, and any notebook notes must be applied for through Freedom of Information

Individuals requiring a copy of a Motor Vehicle Collision Report can access Service Ontario's webpage for specific directions on how to order reports online, in person at any Service Ontario Centre, or by mail directed to the Ministry of Transportation. A certified copy costs $18.00 or an uncertified copy costs $12.00 .

A copy of the information required to be shared under s.200(1)(c) of the Highway Traffic Act, (commonly referred to as the “driver copy”), can still be obtained free of charge by an involved party simply by contacting the responding officer via email to obtain a copy.

For all other requests concerning property damage or loss reports, or witness statements and officer notes from motor vehicle collisions, you can apply online.

You can apply for a copy of a Motor Vehicle Collision Report online  or you can mail in your request. You are required to include a copy of two pieces of valid identification and payment by certified cheque, money order or bank draft.

All reports will be sent through Evidence.com upon completion unless otherwise specified.

Driver Copy "Free of Charge" Available to Drivers involved in a Motor Vehicle Collision
*Please contact the responding officer via email to obtain a copy.

Level 1 - $38.58 (includes GST)

Includes:
-Face of report
-Involved drivers, owners and insurance information
-officer’s brief synopsis of what occurred
-diagram
-information pertaining to any charges laid

Incident Reports:

  • Confirmation that the incident has been reported to this Police Service
    • Incident number, type of incident, date of incident, location where the incident occurred and the name of the victim/complainant.
    • Details of the incident are not provided in a Level One Service search.

Level 2 - $66.15 (includes GST)

Incident Reports:

Includes everything in a Level 1 plus:
• A synopsis of the incident, description of property stolen if listed (including vehicles), status of stolen property, description of damage
• List of persons charged and pending court dates and location.

Motor Vehicle Collision Reports:

Includes everything in a Level 1 plus:
-officer/driver/witness statements (if available)

pursuant to the Municipal Freedom of Information and Protection of Privacy Act

**Level 2 service is not available if there are charges before the court or if the collision is a fatality or a fail to remain

Please Note:
If charges are before the courts and a Level 2 Service is requested, a Level 1 Service will be forwarded and the $66.15 administration fee submitted will not be refunded.
• Any subsequent requests for a Level 2 Service search will be subject to the full $66.15 search fee.
• Provincial offences normally take four to six months for the charge to clear the court system. Criminal charges normally take six to nine months to clear.
• All information other than the face of the Motor Vehicle Accident Report relating to a fatal motor vehicle collision must be requested pursuant to the Municipal Freedom of Information and Protection of Privacy Act. Notebook notes for any incident must be applied under Freedom of Information.
